To prevent the fraudulent use of of customer's credit card the following information is required before an order is shipped:
Information Required when paying by Charge Card
Full Name as stated on your credit card
Full Billing Address ( the address your credit card statements are mailed to )
Phone Number ( the phone number associated with the above address by the credit card issuing bank and the phone company )
Email Address that is NOT from a free email service
Additional Information That Might Be Required
If the shipping address is different from the billing address or if the above information you provided is incomplete or if an order is suspect , to further protect our customers from fraudulent use of their charge cards we require
a faxed signed authorization , a photocopy of the front and back of the credit card , photocopy of a credit card statement to verify the billing address and a photocopy of driver's license or ID.
